If you wish to provide questions for your clients to answer in a rating scale, you can achieve this in your custom form builder. A rating scale is a great tool to use when conducting a survey for customer satisfaction/feedback, for example.
Within your FORM BUILDER, click on the section of fields called FILE UPLOAD, PAYMENTS, & MORE and select RATING.
After you've added the RATING field, it will appear at the bottom of your existing form. If you need to move it to a different place on your form, drag and drop the field where you want it to be.
You can begin adding your questions by clicking into the box that says QUESTION 1 and replacing this text with your question. Repeat this step for the other questions.
If you need to add more than three questions, or if you want to change the rating values, click on the pencil icon toward the top right corner of the rating scale box and click on ADVANCED.
On the ADVANCED page, the first item you can edit is the RATING VALUES. You can change the number of rating values and change the names of the values.
An example of alternative rating values could be "Very Poor, Poor, Neutral, Good, or Excellent".
In the second section, you can edit your QUESTIONS. Here, you can enter in your three questions, and click on NEW QUESTION at the top of the list of questions to add additional questions.
When you've completed editing your rating scale, click on SAVE CHANGES at the bottom of the page.
