When creating a custom form in your control panel, you may want to allow your clients to select between different options. You have the option to choose how your clients provide these selections to you in your custom form.

Radio Buttons are a great choice if you want your client to make one selection like in the example below:

Checkboxes are a great choice if you wish your client to make multiple selections as illustrated below:

To allow your client to make selections first click on the option for FORMS in your control panel and choose FORM BUILDER.

Click on your form name to customize your selected form.

You can add the RADIO SELECT or CHECKBOX option under the ADD STANDARD FIELDS section.

The new field will be added at the bottom, but you can drag this to any location.
